U.S. Air Force Technical Information Questionnaire — Sighting of 19 November 1965

On the evening of November 19, 1965, at approximately 7:45 P.M., a teacher and his wife were driving north on Madison Avenue in Indianapolis, Indiana, when they observed an oblong object hovering in the sky. The witnesses estimated the object was at an altitude of 250 to 300 feet and located near the 2700 block of Madison Avenue. Initially, the witnesses mistook the object for an aircraft preparing to land at the local airport. However, the object began to glow, shifting colors from white to yellow and then to red. The object then darted away at a tremendous speed, heading northwest until it was out of sight. The entire sighting lasted approximately 10 to 12 minutes. The witnesses, who previously considered such reports to be optical illusions or products of imagination, stated that this experience left them with no doubt regarding the reality of the phenomenon. The primary witness, a teacher at Woodview Junior High School with a background in science, emphasized that they were accustomed to dealing in facts rather than fantasies. The sighting was reported to the Indiana State Police on November 19, 1965, and subsequently to Bunker Hill Air Force Base. The U.S. Air Force, through the Foreign Technology Division (FTD) at Wright-Patterson Air Force Base, initiated a formal investigation, requesting that the witnesses complete a technical information questionnaire. Major Hector Quintanilla, Jr., Chief of Project Blue Book, noted in a letter dated January 6, 1966, that the observation remained without solution.
