On 20 November 1966, a civilian witness in Glenside, Pennsylvania, observed an unidentified aerial phenomenon. The witness reported the sighting to Lt. J. E. Seanor at the Naval Air Station (NAS) Willow Grove on the same day. The observation lasted approximately 10 minutes under clear, dry weather conditions. The witness initially described seeing one object at an estimated altitude of 21,000 feet, which subsequently split into multiple objects that formed a V-formation and moved in an eastward direction. The witness completed a U.S. Air Force technical information questionnaire (FTD Form 164) to provide details of the event. On 13 December 1966, Major Hector Quintanilla, Jr., Chief of Project Blue Book, sent a letter to the witness stating that the information received was insufficient for a formal evaluation and requested that the witness complete the provided form to assist in the investigation.
