Relatório Sobre OANI (UAP Report Form)

This document is a comprehensive, standardized reporting form used by the Brazilian Air Force (Força Aérea Brasileira) under the SIOANI program to document sightings of Unidentified Aerial Phenomena (OANI). Section IV is dedicated to technical data regarding the OANI, such as its shape, dimensions, luminosity, and physical features like windows, doors, or support structures. Section V addresses objective evidence, including photography, cinematography, and physical traces at the site. Section VI documents simultaneous events, such as effects on local radio, electrical systems, animals, or human witnesses. Finally, Section VII provides space for a historical description of the research and a final assessment by the Chief of SIOANI.
