Sighting of Unidentified Phenomena Questionnaire — August 1967, Churchville, New York

This document contains a series of correspondence and a formal sighting report regarding an alleged UFO encounter in August 1967. The witness, a park guard employed by Monroe County, New York, reported that while driving his truck in Churchville Park, he encountered a 50-foot-long, cigar-shaped object on the pavement. He described two 'little men' the size of midgets, dressed in black rubber uniforms, who entered the object through a panel. The object then emitted a loud hissing sound and took off straight up. The witness claimed that his report to the local sheriff's department led to his termination and subsequent social humiliation. The witness wrote directly to President Lyndon B. Johnson in January 1968, expressing his frustration that the Air Force had not investigated his report. The Department of the Air Force responded by stating they had no record of the incident and provided a questionnaire for the witness to complete. The official Air Force evaluation concluded that there was 'insufficient data for evaluation' because the report was received too late (February 1968) to conduct a timely investigation. A memorandum for record dated January 29, 1968, confirms that the sighting was not present in the official Blue Book files.
